ASET Forms

The Employee Provisioning Form in the ServiceNow Employee Center will replace the AAA Form for new employee onboarding. Go to ADOA-ASET Service Desk > Quick links > select Employee Provisioning.

Remote Support Portal - Use the Remote Support Portal to grant remote access to Service Desk staff for troubleshooting computer issues, downloading software/updates, and more.

The Cloud Smart Policy Exception Form allows agencies to request an exception to move hardware into an agency IDF or State Shared hosted Data Center. The process will go through an Agency Executive-level approval as well as an ADOA Statewide Executive approval process before hardware can be purchased and installed. Surplus Property Removal - Use this form to submit requests for surplus property removal. Please note: ADOA-managed agencies should submit a ticket to ASET while all other agencies should use this form to coordinate with surplus directly.
